The Registered Nurse Oncology Patient Care Navigator serves as a patient liaison to help patients and family members navigate the complex healthcare system. Functions as a clinical resource to patients, physicians, and the cancer program for assigned cancer site(s)through all phases of cancer trajectory by coordinating a system-wide delivery of service expectations. The RN Patient Care Navigator will establish early contact with cancer patients to identify and eliminate barriers to timely diagnosis and treatment. S/he will assess clinical, emotional, spiritual, psychosocial, financial and other patient needs. Utilizes the nursing process to provide coordinated patient care by actively collaborating with other members of the healthcare team and case managing patients. Provides direct patient and family education, as well as community outreach, education, and screening. The Patient Care Navigator will be responsible to monitor and report on outcome measures. S/he will refer patients to appropriate clinical trials and supportive care services, as well as direct patients and families to available community resources. The Oncology Patient Care Navigator will serve as a clinical resource with expertise in oncology care management in reinforcing education with patients and families.
