Serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations for an extended period of time across the continuum of care for both inpatient and outpatient oncology patients to address disparate cancer-related health outcomes.


Facilitates the coordination of patient care services to assure excellence in patient care and patient flow. Assists patients to eliminate operational barriers to services (such as scheduling, test results, etc.). Works closely with other healthcare disciplines to ensure timely appointments, results reporting, financial needs referrals, communication, patient care, and follow-up.


Contributes to an environment of quality and process improvement and entry into patient databases. Identifies metrics to monitor for specific patient populations and maintains databases and creates reports.


Provides patient and family education and develops educational tools. Plans and implements cancer related educational programs for individuals and groups on topics related to specific cancers including prevention, detection, treatment, and strategies in coping with the disease.


Facilitates the development, implementation and adherence to clinical and research guidelines. Works with treating physician to determine eligibility for research studies and if eligible, coordinates all aspects of requirements of the study.


Interfaces with other healthcare team members for appropriate referrals/services (Oncology Nurse Case Manager, pastoral care, dietitian, Hospice, homecare, etc.)


Provides care of the patient through nursing assessment, planning, implementation, and evaluation.


Organizes, develops, and oversees multidisciplinary conferences. Documents and completes physician recommendations.


Up to two years formal training or education beyond high school level. (e.g. Associate Degree)

Current Colorado or Compact State RN License Oncology Nurse Certification
