To provide indirect and direct medical services to clinic patients. To provide support for the healthcare, providers, office staff and manager. Provides individualized diabetes self-management education/training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators.

The RN CDE works under the supervision of the Practice Administrator, and the direction of the Medical Director(s). This position also assists in managing direct patient care and management.

The RN CDE will be an educator and our reach coordinator for the Diabetic population approximately 70 percent of the time. The other 30 percent will be spent doing general RN duties for the clinic or special projects.

  • Collects DSME/T program participant assessment data, using the AADE7 framework, in a collaborative and ongoing manner

  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T Program participants

  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods

  • Collaboratively develops an individualized follow-up plan with each program participant

  • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives

  • Conducts a follow-up assessment upon completion of DSME/T Program services, using outcome measures from AADE7 Continuum of Outcomes Framework

  • Documents assessment data, educational plan, educational services provided and evaluation results in each participant�s educational records

  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed

  • Identifies when a program participant�s needs are outside the scope of the instructor�s practice and expertise, plus arranges additional services to meet needs

  • Performs clinical skills competently for possible RN duties including:

    • Laboratory results review and dissemination

    • Abnormal lab value reconciliation

    • Medication refills

    • Triage of clinic patients

    • Procedures / patient education as specified by site.

    • Triage phone calls and document advice given and disposition of patient

    • Triage walk-in patients

    • Respond to emergent situations in the clinic

    • Utilized electronic health record

    • Book patient appointments as needed

    • Conduct follow-up phone calls to patients and document in EHR

    • Assist with protocol development

    • Assist with QI activities

    • Participate in clinic team projects

    • Education of patients (i.e. insulin teaching, medicine regimes)

  • Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.

    • Attend required meetings.

    • Complies with safety and infection control procedures as established within the clinic.

    • Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to differentiate the care that is required based on the patients� current place on the life growth and development flow.

    • Documents correctly and accurately in electronic health record.

    • Expresses thoughts clearly and concisely (verbal and written).

    • Follows departmental policies and procedures.

    • Introduces and greets patients in a prompt and pleasant manner.

    • Maintains patient confidentiality.

    • Maintains patient files, records, and other information.

    • Participates in professional development activities and internal/external customer service.

    • Responds to emergency activity within the office according to protocol.

    • Self direct, motivate and function successfully in a team environment.

    • Performs other related duties as appropriate and as assigned.

      Patient Centered Medical Home Duties:

    • Is self-directed, motivated and can function successfully in a team environment

    • Attends required staff meetings

    • Attends required training meetings

    • Participates in clinic and team Quality Improvement processes

    • Involves the PCMH Care Managers with follow through on patient self-management

    • Involved in huddles on daily basis per huddle procedure

    • Involved in team based care

    • Attend monthly meetings at which clinics, PCMH, and best-practice procedures and business agenda are addressed

    • Attend Performance Improvement Team and other QI committees as assigned

  • Current BLS certification

  • Graduate from Accredited Nursing Program

  • Current Nursing License in the State of Colorado

  • A healthcare professional who is also a certified diabetes educator, or who is eligible to become a certified diabetes educator.

  • Minimum of 6 months experience (preferably more) providing diabetes education and/or 15 contact hours within past year in diabetes and diabetes clinical and educational subjects


  • 2 years Registered Nurse experience in Family Medicine office practice.

  • Experience working with, and sensitivity toward, special populations, including those with limited income, and limited education, and with non-English speaking individuals

  • Working knowledge of Microsoft Office programs

  • Full understanding and working knowledge of EPIC computer systems

  • Experience with PC software applications in word processing, Word, and Excel

  • bilingual English/Spanish

    • Graduation from an accredited program for Registered Nurse.

    • Certified diabetes educator

    • Current basic life support certification required.
