SCL Health Medical Group is a physician network, which is part of SCL Health. SCL Health Medical Group provides medical care to individuals and families living in communities throughout the Denver metro and Boulder areas. This network is comprised of more than 40 primary care and specialty clinics and approximately 150 physicians and advanced practice clinicians. SCL Health Medical Group offers convenient options and services for patients and families including personal online access to medical records and provider care teams, extended hours, and multiple locations to ensure you can connect with a provider close to home or at whatever location is most convenient for you. These physicians are part of a nationally recognized, faith-based, health system with nine hospitals and hundreds of providers committed to sharing knowledge and best practices for safe, effective health outcomes. 

Provides independent and advanced professional nursing care through assessment, planning, intervention and evaluation.

  1. Function independently to perform physical assessments of patients discriminating between normal and abnormal findings using sound judgment and critical problem solving skills.  Obtains pertinent medical history. Performs appropriate physical exam. Obtains and reviews relevant laboratory or radiologic studies.  Identifies and assesses significant co-morbidities.  Develops appropriate treatment plans.  Initiates appropriate actions to preserve patient life, foster recovery, and enhance wellness.
  2. Provides efficient and compassionate care for both the patient and their family utilizing multi-disciplinary approach through communication, continuity, coordination and evaluation. Provides relevant education and counseling to patients and families to support care plan, treatment, and discharge.
  3. Collaborates with other healthcare team members, leadership, and medical staff to provide safe patient care.  Communicates pertinent findings and information to physician and team. 
  4. Assists collaborating physicians with management of patients in the hospital consistent with care site policies, regulations, and approved Scope of Practice.
  5. Responsible for compliance of regulatory standards and practice metrics.  Documents in accordance with regulatory and care site guidelines.
  6. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards.
  7. Performs other duties as assigned.
  8. May be required to float to other departments (within scope of competency and qualifications) based on business need.  May be required to be placed on-call during a regularly scheduled shift.
EDUCATION
 
Required:  Graduation from an accredited School of Nursing is required.  Completion of a postgraduate Nurse Practitioner Program accredited by the NLN or ANCC-CCNE is required.

CERTIFICATION, REGISTRATION, LICENSE

Required: Current Registered Nurse license in state of practice is required.  Current certification in state of practice as Nurse Practitioner is required.  Advanced Cardiac Life Support certification is required.

EXPERIENCE
Required:  Two years of RN level work experience is required. 
 
Preferred: 2 - 5 years provider level exp.

Required:  Graduation from an accredited Physician Assistant program is required.
