General Responsibilities
  • Patient care activities to increase the client’s function and independence including:
    1. Evaluation (initial and interim) to determine level of function and problem identification;
    2. Goal setting and care planning in consultation with the physician to achieve goals;
    3. Treatment provision, patient/family education and provision of equipment to increase the patient’s level of function and independence,
    4. Discharge evaluation and planning.
  • Documentation including:
    1. Timely recording of all evaluation data, treatment, and client’s response to therapy interventions.
    2. Recording outcome of conferences and quality assurance data.
  • Attends and participates in appropriate client care conferences, staff meetings, in-service programs and agency committee meetings.
  • Provides continuing education and consultation to clients, families, and other staff members.
  • Supervises Occupational Therapy Assistants (OTA) and Home Health Aides (HHA) as appropriate.
  • Educates staff in clinical aspects, as needed.
  • Maintains current knowledge of the Occupational Therapy (OT) profession by participation in formal education programs, workshops, and professional organizations.
  • Performs other related functions as required.
  • Graduate of an Occupational Therapy curriculum approved by the Council of Medical Education of the American Medical Association and the American Occupational Therapy Association.
  • Two (2) years of experience as a Physical Therapist), preferably in Home Care settings.
  • Thorough knowledge of Occupational Therapy principles and procedures.
  • Working knowledge of possible hazards to clients during treatment and of the necessary precautionary measures.
  • Skills in assessing client needs accurately and setting realistic goals for rehabilitation.
  • Ability to communicate effectively verbally an in writing.
  • Ability to incorporate new ideas or concepts into practice.
  • Ability to establish and maintain effective relationships with professional and non-professional staff members, clients and the general public.
Special Requirements
  • Has completed successfully the National Registration Examination of the American Occupational Therapy Association or is licensed by another state.
  • Valid, current Colorado Driver’s license.
  • Access to a reliable automobile.
